I just downloaded 4.0.0239. I currently have 4.0.0233 installed. Do I need to uninstall my current version first? Or can i just run the new .msi on top of what I already have?

Yes, it's always a good idea to uninstall your existing version first. Otherwise, VS might get confused with multiple instances of our assemblies in the GAC or other places in your hard drive.
