Right now Blend is still work in progress and Microsoft hasn't released any documentation on its designer capabilities yet, which are also still work in progress. So for now, we recommend coding your ribbon UI using the XML Editor in Visual Studio.
Our sample project has plenty of sample XAML code to get you started, especially if you follow along with the many QuickStarts that are included.
Even though it's released, as far as I've seen from searching on the web and whatnot, there is still no documentation on adding designer support to third-party controls. I'll send Microsoft an email to see if they have any new resources they can point us to.
