Title bar - can't see minimize, restore, & close buttons

This doesn't happen all the time, but very frequently, the title bar appears to be solid black instead of transparent and it covers up the minimize, restore, and close buttons. It happens if it's maximized or restored down to a smaller size window. If you change the window size, it goes away and comes back as you resize. I can send a screenshot if that would help.
